29 CFR 1926.651(k)(2): Where the competent person found evidence of a situation that could result in possible cave-in, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions, exposed employees were not removed from the hazardous area until the necessary precautions have been taken to ensure their safety: (a) On or about April 16, 2019 and at times prior to that date, for the employee installing a fire suppression water valve system where ground water was causing or likely to cause cave-in hazards in a trench approximately 5 feet 10 inches deep without an adequate protective system located at the vicinity of NBC 776 BEQ Bldg., Naval Base Coronado, San Diego, California. (b) On or about May 2, 2019, for the employees repairing a fire suppression water line damaged during excavation where water was causing or likely to cause cave in hazards in a trench approximately 5 feet deep without an adequate protective system located at the vicinity of NBC 779 BEQ Bldg., Naval Base Coronado, San Diego, California. (c) On or about May 6, 2019 and at times prior to that date, for the employee installing a fire suppression water line in a trench approximately 5.5 feet deep without an adequate protective system located at the vicinity of NBC 779 BEQ Bldg., Naval Base Coronado, San Diego, California.

29 CFR 1926.652(c)(2)(i): 29 CFR 1926.652(c)(2)(i): Design of support systems, shield systems, or other protective systems that are drawn from manufacturer's tabulated data were not in accordance with all specifications, recommendations, and limitations issued or made by the manufacturer: (a) On or about April 16, 2019 and at times prior to that date, for the employee installing a fire suppression water valve system where ground water was causing or likely to cause cave-in hazards in a trench approximately 5 feet 10 inches deep and the Safe-T-Shore Hydraulic Aluminum Vertical Shoring system was installed in Type C soil using unspecified plywood sheeting to include but not limited to oriented strand board (OSB) located at the vicinity of NBC 776 BEQ Bldg., Naval Base Coronado, San Diego, California. (b) On or about April 16, 2019 and at times prior to that date, for the employee installing a fire suppression water valve system where ground water was causing or likely to cause cave-in hazards in a trench approximately 5 feet 10 inches deep and the Safe-T-Shore Hydraulic Aluminum Vertical Shoring system members not bearing on firm soil located at the vicinity of NBC 776 BEQ Bldg., Naval Base Coronado, San Diego, California. (c) On or about April 16, 2019 and at times prior to that date, for the employee installing a fire suppression water valve system where ground water was causing or likely to cause cave-in hazards in a trench approximately 5 feet 10 inches deep and the Safe-T-Shore Hydraulic Aluminum Vertical Shoring system was installed with horizontal lateral loads applied to the hydraulic cylinders located at the vicinity of NBC 776 BEQ Bldg., Naval Base Coronado, San Diego, California.
