Site - On or about May 7, 2019, employees engaged in material handling while standing directly underneath the scaffolding system were exposed to head injuries from falling or dropping materials,where hardhats were not provided or utilized to prevent the hazard.

Roof scaffolding system - On or about April 18, 2019, employees engaged in chimney activities were exposed to fall hazards where fall protection was not provided or utilized to prevent the fall hazard.        b. Three tier scaffolding system - On or about April 18, 2019, employees engaged in chimney activities were exposed to fall hazards where fall protection was not provided or utilized to prevent the fall of over 12 feet.

Site - On or about April 18, 2019, employees were exposed to fall hazards while navigating the chicken ladder for access to the roof scaffolding system where fall protection was not provided or utilized to prevent the fall hazard.
