29 CFR 1926.503(c)(3): Inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill.    Rooftop: Employee(s) were removing the drip stop edging along the edge of the flat roof. In the following instances, employees were using personal fall arrest systems not in accordance with manufacturer's specifications:   a) Employees were using FallTech full body harnesses with the straps not adjusted properly and the dee-rings were located on their lower backs.   b) Employees were using FallTech full body harnesses with the leg straps loose.   c) An employee was using a Guardian Fall Protection Temper Anchor (Part 00455) that was missing a fastener from the center column.   Employees were exposed to fall hazards of 48 feet to the lower level.

29 CFR 1926.502(f)(1)(ii): When mechanical equipment was being used, the warning line was not erected at least 6 feet (1.8 m) from the roof edge which is parallel to the direction of mechanical equipment operation, and at least 10 feet (3.1 m) from the roof edge which is perpendicular to the direction of mechanical equipment operation.  Rooftop: Employee(s) were using the Varimat air welding machine perpendicularly to the warning flags. At the time of the inspection, the warning lines measured 7 feet from the edge. Employees were exposed to fall hazards.
