Rooftop: Employee(s) were removing the drip stop edging along the edge of the flat roof. In the following instances, employees were using personal fall arrest systems not in accordance with manufacturer's specifications:   a) Employees were using FallTech full body harnesses with the straps not adjusted properly and the dee-rings were located on their lower backs.   b) Employees were using FallTech full body harnesses with the leg straps loose.   c) An employee was using a Guardian Fall Protection Temper Anchor (Part 00455) that was missing a fastener from the center column.   Employees were exposed to fall hazards of 48 feet to the lower level.

Rooftop: Employee(s) were using the Varimat air welding machine perpendicularly to the warning flags. At the time of the inspection, the warning lines measured 7 feet from the edge. Employees were exposed to fall hazards.
