29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person.    Location: Around the House     On or about 5/6/2019, employees were exposed hazards to associated with scaffold failure while walking on a scaffold that was not erected under the supervision of a trained scaffold competent person and deficient in the following areas:     Instances:  1: Pump Jack Scaffold System not Fully Planked from upright to guard rail  2: Pump Jack Scaffold System Plank was greater than 14 inches from the face of working area  3: Pump Jack Scaffold System Plank overhang was greater than 18 inches  4: Pump Jack Scaffold System not secured from displacement at or near the bottom of he pole  5: Roof Bracket Scaffold Plank less than 12 inches wide   6: Roof Bracket Scaffold Plank overhang greater than 18 inches

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed    Location: On the Scaffold    On or about 5/6/2019, employees were exposed hazards associated with ladder failure while accessing the roof from the scaffold where step ladders were closed, leaning on the building, and therefore used in a manner other than the intended ladder design    Instances  1: West side scaffold, step ladder leaned on building and used for roof access  2: East side scaffold, step ladder leaned on building and used for roof access

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501  Location: On the Roof  On or about 5/14/2019, employees were exposed to falls up to 26 feet to the ground while stripping the roofing material off of a residential construction project where fall protection was not utilized.  Instances: 1: West side of Roof 2: East Side of Roof  Mark Hurwitz was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number: 1363034, citation number 1, item number 3 and was affirmed as a final order on February 14, 2019, with respect to a workplace located at 50 Farm Pond Road Oak Bluffs, MA 02557
