Observed on or about 5/6/19.

29 CFR 1926.454(a): The employer did not have each employee who performed work on a scaffold be trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used, and to understand the procedures to control or minimize those hazards.    a) Jobsite: 7035 E Lehigh Ave, Philadelphia, PA, Scaffold - The employer did not train their employees to recognize the hazards that are present when working from supported tubular welded frame scaffolds, such as, but not limited to, fall from heights, and chemical, and to understand the procedures to control or minimize those hazards.    The employer was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 121720, citation number 1, item number 6b, and was affirmed as a final order on 8/21/17, with respect to a workplace located at2515 E Albert St, Philadelphia, PA19125.    Observed on or about 5/6/19.

Employees used hazardous chemicals such as, but not limited to hexavalent chromium, quartz (crystalline silica), etc. contained in Keystone Cement Company's Masonry Cement.
