29 CFR 1910.22(c): The employer did not provide, and ensure each employee used, a safe means of access and egress to and from walking-working surfaces: At the 25-2 conveyor, the employer did not ensure employees used the designated and approved cross-overs to cross over the carrier/conveyor areas while the conveyors were in operation moving horizontal carrying carts. The employees were exposed to falls, caught-in and amputation hazards.

29 CFR 1910.30(c): Retraining. The employer must retrain an employee when the employer has reason to believe the employee does not have the understanding and skill required by paragraphs (a) and (b) of this section. At General Motors facility, the employer did not provide retraining to employees who did not obtain the knowledge and understanding to utilize approved crossing locations on operating conveyors to prevent employees from being exposed to falls, caught-in and amputation hazards.
