The 20 foot extension ladder was not inspected by the company to identify a damaged anti slip safety shoe.

29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service and were placed in service when the examination showed condition(s) that would adversely affect the safety of the vehicle. Examinations were not made at least daily:    Employees engaged in operating powered industrial trucks to move materials around the facility located at 401 Miller Drive, Wamego, Kansas were exposed to struck-by hazards due to the following industrial trucks not being inspected daily.  Powered industrial trucks were in service and had the following deficiencies:    (a)  Caterpillar #GP30 (#5) had no functional horn, missing seat belt connector, and hydraulic hose indicating structural damage.    (b)  Yale #6 had no seat belt nor a functional backup alarm.    (c)  Snorkel lift 41G had no functional horn.

29 CFR 1910.212(a)(1):  One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     (a)  Employee(s) engaged in operating the V - 140 bridge mill located at 401 Miller Drive, Wamego, Kansas were exposed to struck-by hazards as a result of the employer's failure to ensure that the machine shuts off when the access door is opened during operation, thus exposing the operator to rotating parts and flying debris created during operation.    (b)  Employee(s) engaged in operating the three T - 30 horizontal mills located at 401 Miller Drive, Wamego, Kansas were exposed to struck-by hazards as a result of the employer's failure to ensure that the machine shuts off when the access doors are opened during operation, thus exposing the operator to rotating parts and flying debris created during operation.    (c)  Employee(s) engaged in operating the T - 10 mill located at 401 Miller Drive, Wamego, Kansas were exposed to struck-by hazards as a result of the employer's failure to ensure that the machine shuts off when the access doors are opened during operation, thus exposing the operator to rotating parts and flying debris created during operation.    (d)  Employee(s) engaged in operating the E420 machine in the south shop located at 401 Miller Drive, Wamego, Kansas were exposed to struck-by hazards as a result of the employer's failure to ensure that a south side panel door approximately 19 inches by 64 inches was in place to prevent the operator from rotating parts and flying debris created during operation.
