29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, nor by any of the means listed in subparagraphs (A) through (D) of this paragraph:     a) North side of building, Unit #13, on or about 5-8-19:  One employee was pressure washing smoke residue from a brick wall using a metal spray wand, holding the wand within 16 inches of an unprotected energized service entrance.       b) North side of building, Unit #13, on or about 5-8-19:  One employee used his bare hands to remove plastic sheeting that had been draped over an unprotected energized service entrance, with his hands placed within 4 inches of the live line.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:    a) North side of building, Unit #13, on or about 5-8-19:  One employee was working on a "pick" type scaffold platform that was not inspected by a competent person, in that the platform ends were setting on the surface of two entry canopy roofs, exposing the employee to a fall hazard of 12 feet 9 inches to the pavement below.
