29 CFR 1915.73(d): When employees were exposed to unguarded edges of decks, platforms, flats, or similar flat surfaces, more than 5 feet above a solid surface, the edges were not guarded by adequate guardrails meeting the requirements of 29 CFR 1915.71(j)(1) and (2).    Shop bay one Mezzanine; on or about 08 May 2019 and times prior thereto; the employer did not ensure that unguarded edges were effectively protected. An unguarded edge measuring approximately 126 inches above the lower surface did not have guardrails installed. This condition exposed employees to a fall hazard.

29 CFR 1915.81(b)(1)(iii): The employer did not ensure that each walkway was kept clear of tools, equipment, materials, and other objects that created a hazard for employees.    Shop bay stairs; on or about 08 May 2019 and times prior thereto; the employer did not ensure that each walking and working surface was kept clear of hazards. The stairway had miscellaneous items stored on the stair steps. This condition expose employees to a tripping hazard.

Shop bay 1; on or about 08 May 2019 and times prior thereto; a bench mounted grinder, make and model unknown, did not have the work rest adjusted to less than one-eighth of an inch from the abrasive wheel. This condition exposed employees to a caught in hazard.

Shop bay 1; on or about 08 May 2019 and times prior thereto; a bench grinder, make and model unknown, did not have the protective guard installed. Angular exposure exceeded 180 degrees.  This condition exposed employees to a caught in hazard.

29 CFR 1915.507(b)(1): The portable fire extinguishers and hose systems were not selected, installed, inspected, maintained, and tested by the employer according to NFPA 10-2002 Standard for Portable Fire Extinguishers.  Shop bay; on or about 08 May 2019 and times prior thereto; portable fire extinguishers were not inspected in accordance with NFPA 10-2002 Standard for Portable Fire Extinguishers. Two instances were noted;       a. An Amerex model B441, serial number TF-288780. Last inspection date indicated was August 2011.       b. An Amerex model B456, serial number B-76248028. Last inspection date indicated was August 2016.  This condition exposed employees to fire hazards.
