29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.   Site- Employees were not protected from a fall hazard where there was no access provided between a wood plank scaffold in the awning and a tube and coupler scaffold tower built from the ground. employees were climbing between the two scaffold platforms where the horizontal distance between the platforms was greater than 3 feet.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    Site- Employees were not protected from a fall hazard where a Werner 8 foot step ladder was used to access the work platform of a scaffold tower and the top of the ladder was not secured to prevent displacement where the ladder was not long enough to extend three feet above the landing surface.
