29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) Northwest side of roof, on or about 5-8-19: Three employees were performing roofing on a sloped roof, 7:12 pitch, without the training necessary for the employees to recognize the hazards associated with falls and their work, exposing the employees to a fall hazard of 24 feet to the pavement below. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) North side of building, on or about 5-8-19: Four employees were using a portable aluminum extension ladder for roof access/egress, with side rails that were 18 inches above the landing surface, exposing the employees to a fall hazard of 9 feet 6 inches to a lower roof surface. b) North side of building, on or about 5-8-19: Four employees were using a portable aluminum extension ladder for roof access/egress, with side rails that were 22 inches above the landing surface, exposing the employees to a fall hazard of 11 feet 6 inches to the pavement below.
