On or about 5/08/2019, Luis Pene DBA Luis Pene was removing and installing roofing materials on a roof of a residential building located at 75 Casey Court in Wentzville, MO. Two employees were installing shingles with pneumatic nail guns and one employee was cutting a ridge vent with a circular saw. None of the employees were wearing eye protection.

On or about 5/08/2019, Luis Pene DBA Luis Pene was removing and installing roofing materials on a roof of a residential building located at 75 Casey Court in Wentzville, MO. The owner and four employees were not utilizing guardrail systems, a safety net system, or personal fall arrest systems while working near the edge of a roof approximately seventeen to twenty-four feet above the ground.
