29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: On or about 05/07/2019, Builtech Services, LLC (General Contractor), working at a commercial site located at 1680 Hwy K, OFallon, MO, directed one of their subcontractors employee(s) to break concrete at or near an electrical energized conduit line. The General Contractor failed to have the electrical line(s) deenergized nor were any other special precautions taken. An incident occurred which caused an electrical flash and disrupted the power.
