29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  (a) Osvaldo Vega, on or about May 9th, 2019, at the location of 2227 Drummond Rd., Toledo, OH, 43606: The employer did not have an accident prevention program or plan to identify job site hazards which assists employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury or illness.

Employees performing work in this condition were exposed to falls ranging from 9 feet to 18 feet to the ground below.
