29 CFR 1926.501(b)(10):  Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, warning line system and personal fall arrest system, or warning line system and safety monitoring system; or, on roofs 50 feet (15.25 m) or less in width, the use of a safety monitoring system alone:   a) Southeast wing of building, 36 Old Hickory Ridge Road, Washington, PA 15301:  On or about May 7, 2019, employees working on a flat roof on the southeast wing of a three story building were not protected from falling.   The middle section of the building was approximately 45 feet in width.  The employees were exposed to a fall hazard of approximately 30 feet to the ground below.    b) Middle section of building above main entrance, 36 Old Hickory Ridge Road, Washington, PA 15301:  On or about May 7, 2019, employees working on a flat roof in the middle section of a three story building were not protected from falling.   The middle section of the building was approximately 75 feet in width.  The employees were exposed to a fall hazard of approximately 30 feet to the ground below.
