On 3/12/2019, employees of RCA Roofing and Construction, Inc. were working on a residential building located at 1247 Creve Coeur Crossing Lane, Chesterfield, MO. Employees working on the roof were replacing plywood roof decking which created unprotected holes in the floor. Employees were not protected from through the holes to the lower level by personal fall arrest systems, covers, or guardrail systems around the holes.

Employees were installing shingles approximately 24 feet above a lower level. Employees were not utilizing guardrail systems, a safety net system, or a personal fall arrest system. The employees were observed on the roof of the building without fall protection.
