Or, on roofs 50-feet (15.25 m) or less in width, each employee is not protected by use of a monitoring system:  (a)  The employer is failing to ensure that employees are protected from fall hazards of 6 feet and greater during roofing work.  This was most recently observed on May 2, 2019 when employees of JCO Roofing LLC were exposed to fall hazards while performing roofing work without the use of fall protection systems on a roof in excess of six feet above the lower surface.    JCO Roofing, LLC was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number 1361484, Citation Number 2, Item Number 1, and was affirmed as a final order on February 12, 2019, with respect to a workplace located at 4480 Malibu Point, Colorado Springs, CO.  JCO Roofing, LLC was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number 1352092, Citation Number 1, Item Number 1, and was affirmed as a final order on November 13, 2018, with respect to a workplace located at 4980 Broadmoor Bluffs Dr., Colorado Springs, CO.
