29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. Location: 5300 4th Ave. On or about: May 8th 2019 a) Employee was painting a flange attached to a concrete column for the MTA subway system at street level and was exposed to a fall of more than 20 feet.
