On or about, 5/7/2019, at 126 S. Port Road, Albany, NY 12202. For employees hired to work as temporary workers for Spartan Materials LLC to clean debris. The staffing agency did not conduct a walkaround at the site to assess the workplace to ensure that an N95 was the required PPE to conduct the work safely.
