A) Geo Gradel Co. on or about May 6, 2019 at the address of 28625 Tracy Road, Rossford, OH: The employer did not ensure a competent person conducted daily inspections of the excavation / trench to ensure that employees were adequately protected from hazards associated with trenching work. The need for adequate and effective cave-in protection was not evaluated or recognized prior to permitting employees in a trench excavation.
