29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person Location: Harborview Hotel, Mayhew Building, N.Water St. Edgartown, Martha's Vineyard, MA On May 6th, 2019, employees were exposed to falls hazards, struck by hazards, electrical shock hazards, and caught between hazards where frequent and regular inspections made by a competent person did not identify and mitigate the hazards: Instances: a) Scaffold above the entryway in the rear of the building was inadequately erected and not inspected by a competent person. b) Siders and framers were working above the entryway and around the exterior of the building and head protection was not utilized throughout the project c) The exterior decks used as an entryway on the 2nd and 3rd floor in the rear of the building did not have guard rails installed d) Temporary lighting inside the building was missing bulbs e) Fire extinguishers were not inspected and/or inoperable in the building
