29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system:  Jobsite: The employee standing on the roof and then in the ceiling space to fix the roof leak was not provided with appropriate fall protection systems/devices.  On or about March 22, 2019, the employee fell approximately 7 feet on to the ceiling joists and then another 9 feet onto the ground level while performing the above task. The employee sustained minor injuries from the fall.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  Jobsite:  The employee exposed to fall hazards approximately 16 feet to the ground level while working inside a ceiling area on rafters to repair a roof leak was not trained in fall hazards and the procedures to be followed to minimize such hazards.
