29 CFR 1910.106(b)(6): In locations where flammable vapors may be present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition.  Sources of ingition may include open flames, lightning, smoking, hot sirfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat.    On or about May 9, 2019, in the HAWG SWD tank farm, an employee was using a Tsurumi (Model EPT3-50HA) gasoline powered pump in a location where flammable vapors were present; this condition exposed the employee to explosion and burn hazards.
