29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Supervalu, Inc., 3900 Industrial Road, Harrisburg, Pennsylvania - The lockout/tagout procedures were not developed for performing servicing and maintenance work, when replacing a counterweight strap on Rytec door, serial number D0055129-240, maintenance employees did not turn off power and apply lockout/tagout this piece of machinery, exposing the employees to caught in and struck by hazards, on or about May 14, 2019. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) Produce/Perishables - Two maintenance employees worked on the warm side of the Rytec overhead door, serial number D0055129-240, to replace the broken counterweight strap on the left side facing this Rytec door. These employees did not lockout/tagout the electrical disconnect for the door while doing this servicing and maintenance work, exposing the employees to caught in and struck by hazards, on or about May 14, 2019. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
