29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: A) Mansfield Plumbing Products, Inc., located at 150 East First Street, Perrysville, OH 44864: On or about May 7, 2019, the employer did not ensure that one or more methods of adequate machine guarding were provided to protect employees from the hazards such as, but not limited to, pinch points between the moving arms and stationary rails of the MARQ automated tape machine # 1 (Model # HPR-MF/RH/AB; Serial # 099259) located in the Grade & Pack Department. Employees were exposed to caught-in-between and amputation hazards.

Employees were exposed to caught-by and laceration hazards.
