29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a) On May 7, 2019 employees who perform servicing, maintenance and/or cleaning on or around the Staley Rotary Die Cutter were not required to: (1) Turn off and shut down the Staley Rotary Die Cutter line and/or rollers. (2) Physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source. (2) Affix a lockout or tagout devices to the electrical disconnect. In that on May 7, 2019 while cleaning the Staley Rotary Die Cutter, it was not locked out to prevent employees exposure to rotating parts/ pinch point hazards.
