29 CFR 1910.22(d)(2): Hazardous conditions on walking-working surfaces are corrected or repaired before an employee uses the walking-working surface again. If the correction or repair cannot be made immediately, the hazard must be guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired. The employer is failing to ensure that the walking working surfaces employees utilize at or near the gate between sales, and warehouse is maintained and that hazards are corrected and or guarded to prevent injury to employees. This was most recently observed on May 14, 2019 at Peterbilt of Norfolk, 2001 S. 13th Street Norfolk, NE 68701.

29 CFR 1910.178(l)(4)(i): Refresher training, including an evaluation of the effectiveness of that training, shall be conducted as required by paragraph (l)(4)(ii) to ensure that the operator has the knowledge and skills needed to operate the powered industrial truck safely.
