29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury Location: Southend Farm Estates, Lot #8 Frontier Ln., Millis, MA 02054 On or about May 14, 2019, an employee was exposed to a struck-by hazard, where the employee's arm became trapped, while attempting to reposition the thumb attachment of an excavator, as a result of the employer not instructing the employee in the recognition and avoidance of the unsafe condition(s).
