On or about May 9, 2019, Building 700, employees installing roofing shingles on a 4:12 pitch roof were not protected from falling 30 feet to the ground by the use of a fall protection system. b. On or about May 9, 2019, Building 1200, employees tearing off an old roof system on a 4:12 pitch roof were not protected from falling 30 feet to the ground by the use of a fall protection system.
