29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    a. On or about May 9, 2019, Building 700, employees operating nail guns to install roofing shingles were not wearing eye protection and being exposed to flying debris.

29 CFR 1926.501(b)(13): 29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:    a. On or about May 9, 2019, Building 700, employees installing roofing shingles on a 4:12 pitch roof were not protected from falling 30 feet to the ground by the use of a fall protection system.    b. On or about May 9, 2019, Building 1200, employees tearing off an old roof system on a 4:12 pitch roof were not protected from falling 30 feet to the ground by the use of a fall protection system.

29 CFR 1926.1053(a)(7): Ladders not specifically designed for such use were tied or fastened together to provide longer sections:    a. On or about May 9, 2019, backside of Building 700, employees had tied together Werner ladders, one 32-foot extension ladder and one 24-foot extension ladder to provide a longer ladder to access and exit the roof, exposing employees to a 30 feet fall hazard.
