At the worksite located at 1904 Le Compte Road #12, Springfield, Missouri 65802, Employees were exposed to serious struck-by injuries from the Crown forklift serial number 9A118790, which was in operation. The Crown forklift was missing the anti-slip cover on the gas pedal and was in need of repair.

29 CFR 1910.305(j)(2)(iv): A receptacle installed in a wet or damp location was not suitable for the location: At the worksite located at 1904 Le Compte Road #12, Springfield, Missouri 65801, employees were exposed to serious electrical hazards. An electrical receptacle which was installed near the industrial forklift battery refill and emergency eye wash/shower was not suitable for the wet or damp location.
