29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  (a)  On or about May 7, 2019, and at times prior thereto, workers engaged in roofing and were exposed to fall hazards from approximately 10 feet at the eave and up to 26 feet at the peak. The roof pitch was 6:12.  All workers were on roof with no personal fall arrest system.  NOTE: Certification of corrective action is required for this item.

29 CFR 1926.503(a)(1): A training program was not provided to each employee who might be exposed to fall hazards, to enable that employee to recognize the hazards of falling, and be trained in the procedures to be followed to minimize these hazards:  (a)  On or about May 7, 2019, and at times prior thereto, workers engaged in roofing and exposed to fall hazards from 10 feet at the eave to 26 feet at the peak. The roof pitch was 6:12.  All workers were on roof with no personal fall arrest system. Furthermore, the employer did not communicate the hazards or have a fall protection plan to train employees in the use of fall arrest systems.  NOTE: Certification of corrective action is required for this item.
