29 CFR 1910.95(g)(8)(ii)(B): Unless a physician determines that the standard threshold shift is not work-related or aggravated by occupational noise exposure, the employer did not ensure, when a standard threshold shift occurs, that for employees already using hearing protectors, employees be refitted and retrained in the use of hearing protectors and provided with hearing protectors offering greater attenuation if necessary: a) This violation was discovered on or about May 14, 2019, where a production employee experienced a work-related standard threshold shift of 10.6 decibels on his left ear was not refitted or retrained in the use of hearing protectors and provided with hearing protectors offering greater attenuation if necessary. b) This violation was discovered on or about May 14, 2019, where a production employee experienced a work-related standard threshold shift of 14.3 decibels on her left ear was not refitted or retrained in the use of hearing protectors and provided with hearing protectors offering greater attenuation if necessary. c) This violation was discovered on or about May 14, 2019, where a production employee experienced a work-related standard threshold shift of 19.6 decibels on his left ear was not refitted or retrained in the use of hearing protectors and provided with hearing protectors offering greater attenuation if necessary. d) This violation was discovered on or about May 14, 2019, where a production employee experienced a work-related standard threshold shift of 10.3 decibels on his right ear was not refitted or retrained in the use of hearing protectors and provided with hearing protectors offering greater attenuation if necessary. e) This violation was discovered on or about May 14, 2019, where a production employee experienced a work-related standard threshold shift of 13 decibels on his left ear was not refitted or retrained in the use of hearing protectors and provided with hearing protectors offering greater attenuation if necessary.

29 CFR 1904.10(a): Where an employee's hearing test (audiogram) revealed that the employee had experienced a work-related Standard Threshold Shift (STS) in hearing in one or both ears, and the employee's total hearing level was 25 decibels (dB) or more above audiometric zero (averaged at 2000, 3000, and 4000 Hz) in the same ear(s) as the STS, the employer did not record the case on the OSHA 300 Log: a) This violation was discovered on or about May 14, 2019, in the production facility, where an employee experienced a standard threshold shift of 14.3 decibels on her left ear and an average threshold hearing level of 28.3 decibels on the same ear and the employer failed to record the case in the OSHA Form 300 for 2019. b) This violation was discovered on or about May 14, 2019, in the production facility, where an employee experienced a standard threshold shift of 19.6 decibels on his left ear and an average threshold hearing level of 55 decibels on the same ear and the employer failed to record the case in the OSHA Form 300 for 2019.
