29 CFR 1926.454(a)(1): Employer did not ensure that employees while on a scaffold are trained by a qualified person on ways to minimize hazards in the work area. (a) On or about May 8, 2019, and at times prior thereto, worker engaged in using an aerial lift with no fall protection and elevated to heights of approximately 60 to 80 feet above ground level, did not demonstrate an understanding of how to mitigate the hazards associated with the aerial lift, fall hazards and/or fall object hazards in the work area.
