The Hoffman and Harpst Company, on or about May 15, 2019, at the address of 1270 Conant St. Maumee, OH; The employer did not ensure employees were adequately protected from falls while installing a HVAC unit atop a commercial structure. Employees performing this task were exposed to falls up to 18 ft. to the asphalt below.
