29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: a) Country Pine & Sawmill, LLC: On May 13, 2019 and at times prior, the employer failed to report an amputation of one employee as a result of a work-related incident, within (24) twenty-four hours. An employee sustained an amputation of three fingers on May 8, 2019 and OSHA was notified by the employer on May 10, 2019.
