Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a. On or about May 16, 2019, on the 4:12 pitch hotel roof, 13 roofing employees re-roofing the building were exposed to 26 feet to 31 feet fall hazards in that they were not protected by a fall protection system

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a. On or about May 16, 2019, at the west wing of the hotel, the roof access ladder from the ground to the first-story roof was not extended at least 3 feet above the roof edge, exposing employees to an 11-feet and 2-inches fall hazard. b. On or about May 16, 2019, at the west wing of the hotel, the access ladder from the first-story roof to the third-story roof was not extended at least 3 feet above the edge of the roof, exposing employees to a 14 feet fall hazard.
