29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.   On or about May 15, 2019, the employer did not ensure that a guardrail system was installed along all open sides and ends of the scaffold.  Employees were working from the second level of a scaffold that did not have the proper guard rails installed.  This exposed employees to a fall of greater than 10 feet to the ground.
