Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  (a) On or about 05/15/2019 - Lot #145 at Ginn Point, Owens Cross Roads, AL, employees were installing new shingles on a roof with a 4:12 slope at approximately 10 feet above the ground while exposed to a fall hazard.
