29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  On or about 5/14/2019 at 1605 Church Street Decatur, GA; where the employer failed to provide a ladder for workers performing masonry activities at 7-feet above the ground from the fabricated frame scaffold. Workers were observed scaling the frame of the scaffold for access and egress exposing them to fall hazards up to 7-feet.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  On or about 5/14/2019 at 1605 Church Street Decatur, GA; where the operator of the Genie, S-65 lift was observed exposed to fall hazards while not wearing personal fall protection while in the basket performing masonry activities at 32-inches off the ground. The wall that was being worked on would have required the operator to raise to approximately 28-feet.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  On or about 5/14/2019 at 1605 Church Street Decatur, GA; the operator of the aerial lift scaffold failed to ensure the operator was trained to safely operate the Genie, S-65 lift where the operator was observed exposed to fall hazards while not wearing personal fall protection while in the basket performing masonry activities at 32-inches off the ground. The wall that was being worked on would have required the operator to raise to approximately 28-feet to complete the job.
