Location: 626 5th Avenue, Brooklyn, NY. a) Employee worked next to an elevator shaft on the third floor and fell 38 feet when the guardrail systems were removed to transport materials in the shaft. On or about 05/14/2019. The Atlas Developers LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(14), which was contained in OSHA inspection number1224210, citation number 1, item number 1 and was affirmed as a final order on 08/02/2017, with respect to a workplace located at 618 5th Avenue, Brooklyn, NY 11215.
