29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment was isolated from the energy source and rendered inoperative:    a) Sedita Manufacturing, Inc. at 11101 E 53rd Ave, Unit D, Denver, CO: On or about 5/17/2019, the employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment was isolated from the energy source and rendered inoperative. Employees clean and perform maintenance on multiple pieces of equipment, including but not limited to: the Biesse Sektor 470 panel saw, the Holz-Her Pro Master 7123 CNC router machine, and the Holz-Her 1243 sliding table saw. This condition exposed employees to laceration and amputation hazards.

29 CFR 1910.213(c)(1): Each circular hand-fed ripsaw was not guarded by a hood which completely enclosed that portion of the saw above the table and that portion of the saw above the material being cut:    a) Sedita Manufacturing, Inc. at 11101 E 53rd Ave, Unit D, Denver, CO: On or about 5/17/2019, each circular hand-fed ripsaw was not guarded by a hood which completely enclosed that portion of the saw above the table and that portion of the saw above the material being cut. An employee was operating the Holz-Her 1243 sliding table saw while ripping without a guard. This condition exposed employees to laceration and amputation hazards.

Employees operate a Holz-Her 1243 sliding table saw and a Delta Unisaw daily in the production of furniture. This condition exposed employees to laceration and struck-by hazards.

29 CFR 1910.22(a)(2): (2) The floor of each workroom was not maintained in a clean and, to the extent feasible, in a dry condition:  a) Sedita Manufacturing, Inc. at 11101 E 53rd Ave, Unit D, Denver, CO: On or about 5/17/2019, the employer did not ensure that the floor of each workroom was maintained in a clean and dry condition. 15 employees participate in woodworking activities in the production area which produces wood dust. The area around the dust collector in facility had accumulated wood dust which tested to be explosive. The surrounding area housed two large machines which were not spark proof. These conditions exposed employees to fire hazards.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  a) Sedita Manufacturing, Inc. at 11101 E 53rd Ave, Unit D, Denver, CO: On or about 5/17/2019, the employer did not verify that the required workplace hazard assessment had been performed through a written certification. The employer has implemented work rules for the use of some PPE. However, employees exposed to eye hazards were not wearing safety glasses while participating in woodworking activities. Additionally, a workplace assessment had not been performed and certified by the employer. Employees are exposed to struck-by and cutting hazards while working in the production area.
