29 CFR 1910.219(e)(3)(i): Vertical or inclined belt(s) were not enclosed by guard(s) conforming to the requirements specified at 29 CFR 1910.219(m) and (o):    (a)PBM Graphics, Inc. at 604 West Highway 34, Howards, SD 57349: On or about and at times prior to May 9, 2019, the employer did not ensure that employees were protected from pinch-points when working in the vicinity of the conveyor system on the shrink wrap line junction site between 501 and 502. The guard over the inclined sprocket and chain was not adequately secured to the conveyor and duct tape had been placed over a damaged portion of the guard that did not adequately protect employees from pinch-point hazards associated with movement of the chain and sprocket. This condition exposed employees to pinch point hazards.
