29 CFR 1915.13(b)(4): Testing was not conducted by the competent person as often as necessary during cleaning or cold work to assure that air concentrations were below 10 percent of the lower explosive limit and within the Permissible Exposure Limit (PEL) and below Immediately Dangerous to Life and Health (IDLH) levels. NOTE: Factors such as, but not limited to, temperature, volatility of the residues and other existing conditions in and about the spaces shall be considered in determining the frequency of testing necessary to assure a safe atmosphere: In the Barge DBL134, cargo tank #2, starboard side, on May 14, 2019, adequate testing was not conducted to verify the absence of flammable atmosphere in the heating coils prior to work activity.

29 CFR 1915.15(a): Pipelines that could carry hazardous materials into spaces that have been certified "Safe for Workers"� or "Safe for Hot Work"� were not disconnected, blanked off, or otherwise blocked by a positive method to prevent hazardous materials from being discharged into the space: In the Barge DBL 134, cargo tank #2, starboard side, on May 14, 2019, the employer failed to verify the heating coil piping was free of hazardous materials prior to employees work activity.
