Harbor Road, Port Clinton, OH: The employer did not ensure the scaffolding platform used to elevate employees via forklift, was designed and/or evaluated by a qualified person. The platform width exceeded 10 inches beyond the wheel base of the lift, increasing the probability of the forklift overturning. Employees performing roofing activities from the platform in this condition were exposed to falls of approximately 9 feet.
