29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  Worksite: The employer did not ensure that the employee required to perform roofing work on the roof of a commercial building was protected from falling through unguarded skylights by the use of appropriate protective systems/devices.     On or about 05/16/2019, the employee performing roofing work on the above roof fell approximately 24 feet to a lower level through one of unguarded skylights and sustained minor injuries from the incident.
