29 CFR 1910.303(g)(1)(i): Working space for equipment likely to require examination, adjustment, servicing, or maintenance while energized shall comply with the following dimensions, except as required or permitted elsewhere in this subpart: (a)United States Postal Service at 119 Center Street East, Madison, SD 57042: On or about and at times prior to May 14, 2019, the employer did not ensure employees were protected from electrical hazards in that clearance around the electrical panel B-1 was less than that specified in Table S-1. Clearance space to the B-1 electrical panel in the basement was impeded by vehicle tires stored in front of the panel. This condition exposed employees to the hazard of electrical shock and burns. Abatement Note: (A)The depth of the working space in the direction of access to live parts may not be less than indicated in Table S-1. Distances shall be measured from the live parts if they are exposed or from the enclosure front or opening if they are enclosed; (B)The width of working space in front of the electric equipment shall be the width of the equipment or 762 mm (30 in.), whichever is greater. In all cases, the working space shall permit at least a 90-degree opening of equipment doors or hinged panels; and (C)The work space shall be clear and extend from the grade, floor, or platform to the height required by paragraph (g)(1)(vi) of this section. However, other equipment associated with the electrical installation and located above or below the electric equipment may extend not more than 153 mm (6 in.) beyond the front of the electric equipment.

29 CFR 1910.305(b)(1): Unused openings in boxes, cabinets, or fittings were not effectively closed: (a)United States Postal Service at 119 Center Street East, Madison, SD 57042: On or about and at times prior to May 14, 2019, the employer did not ensure unused openings in an electrical junction box were effectively closed. Two knockout plugs were missing on the junction box between the scale and the west bay door in the vestibule, exposing live electrical parts. This condition exposed employees to electrical shock and burns.

Self-luminous or electroluminescent signs that have a minimum luminance surface value of at least .06 footlamberts (0.21 cd/m2) are permitted: (a)United States Postal Service at 119 Center Street East, Madison, SD 57042: On or about and at times prior to May 14, 2019, the employer did not ensure employee were protected from hazards associated with emergency egress in that the exit signs in the Postmasters office were not illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source or have a minimum luminance surface value of at least .06 footlamberts for electroluminescent signs. This condition exposed employees to hazards associated with emergency egress. (b)United States Postal Service at 119 Center Street East, Madison, SD 57042: On or about and at times prior to May 14, 2019, the employer did not ensure employee were protected from hazards associated with emergency egress in that the exit sign on the east side of the lobby was not illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source or have a minimum luminance surface value of at least .06 footlamberts for electroluminescent signs. This condition exposed employees to hazards associated with emergency egress.

This condition exposed employees to hazards related to asbestos exposure.
