On or about May 13, 2019, a water treatment plant employee was required to open sump pits to physically clear a clogged pipe or repair an alarm float. The pits were permit-required confined spaces, and the employer did not evaluate or identify the permit-required confined spaces in the facility or take effective measures to prevent its employees from entering the permit spaces.

On or about May 13, 2019, at the southeast side of Hanger 9, employees were not warned of the danger posed by the permit-required confined space by posting danger signs or by any other equally effective means. c. On or about May 13, 2019, at the rear of Hanger 9, employees were not warned of the danger posed by the permit-required confined space by posting danger signs or by any other equally effective means.
