29 CFR 1910.30(b)(1): (A) The employer did not train each employee on or before May 17, 2017 in the proper care, inspection, storage, and use of equipment covered by this subpart before an employee uses the equipment. On or about May 13, 2019, and times prior thereto, the employer did not train employees on the safe use of the ladders. Employees will use ladder for minor maintenance work and to gain access to the roof. Employees exposed to a fall hazard. The employer has such ladders as, but not limited to: 1. Werner fiber glass step ladder. 2. Metal extension ladder. 3. Metal step ladder.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: On or about May 13, 2019, the employer failed to notify the Occupational Safety and Health Administration (OSHA) when the employee sustained a severe injury and was admitted into the hospital. OSHA became aware of the passing of the employee (owner) on May 18, 2019 when he subsequently died.
