An employee fell to the ground and was injured when they stepped off the scaffold without fall protection.

Instance b) The employer failed to ensure that fall protection was used by employees using a Genie S-45 aerial lift were trained to the nature of fall hazards as a result of the condition of the scaffold that had a lack of fall protection.
