29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Plant: In the following locations, adequate safeguarding was not provided on the following machines: A. East Room: On, or about, May 21, 2019, an employee was seriously injured when he fell into Machine # 1024, an O-M Ltd. Omega 80 CNC VTL (Vertical Turret Lathe). The interlock devices provided on the access doors were not operational. B. East Room: On, or about, May 21, 2019, the interlock devices provided on the access doors for Machine # 1025, O-M Ltd. Omega 80 CNC VTL (Vertical Turret Lathe), were not operational. C. North Room: On, or about, May 28, 2019, the auger-type chip conveyor provided on the Doosan 4106 CNC milling machine was not interlocked with the access door and could run with the door open. D. Front Room: On, or about, May 28, 2019, the access doors provided on Machine #1027, an HNK CNC VTL (#2), were not interlocked. E. Front Room: On, or about, May 28, 2019, the access doors provided on Machine #1026, HNK CNC VTL (#1), were not interlocked. F. Front Room: On, or about, May 28, 2019, the access door provided on Machine #4321, a Patriot VMC-4321 was not interlocked. G. Front Room: On, or about, May 28, 2019, the access door provided on Machine # 5165, a Patriot VMC-4321, was not interlocked.
